Quality improvement tracking

We are a large health system with many sections of our pharmacy dept. Recently had a visit from CMS and we do many QI projects, but it is scattered b/t Ambulatory, Clnical. Inpatient operations, cancer center, etc, with each doing their QI based on issues that need improvement or triggers they recognize as potential problems. We scampered to pull together one database that documented what, when, where, findings, actions taken, reassessment, etc for the department as a whole. Does anyone use a centralized method or software for keeping track of/ organizing all you are doing within your department, methodology, dates, and final actions that resolved or improved a process?
