High Alert Medication List- Pediatrics

We are starting the process of looking at our high-alert medication list since our last update a few years ago. I would appreciate any information (list or policy) that other pediatric institutions have and any strategies your institution uses if a double check process is not required around a high alert medication. Thanks!
