New Order/Same Med Container & Pump Integration

I am looking to connect with others whose organizations have undergone or are in the process of planning for integration. My organization is pursuing this with Epic/Baxter, but I imagine a potential issue we are discussing is widely applicable.

How have you or will you mitigate risk(s) for continuous infusions in a situation in which the same medication bag/syringe is used when a new order is being associated with the pump (e.g., provider modifies the dose on the infusion and RN has a new order to document against in the MAR)? The EHR will send the full volume of that container to the infusion pump despite the actual bag/syringe having less volume.

Does the person administering the med adjust the volume to be infused to match the container already hanging after associating the order with the pump? Does your organization require a new medication bag/syringe for any new order? Have you identified or implemented a different solution?

I hope someone has this figured out and can provide needed insight!
